Use this checklist to identify any areas of Change Management that have not been entirely successful. Then reinforce them by revisiting and re- implementing the relevant section of the FITS process.

Request for change forms are completed for all major changes and the content enables approvers to understand and authorise the changes.

Change management reports are produced regularly.
Change management reports are used to understand the use of the process, identify issues and make decisions.
